How it works
Identity is checked
Claimant name and date of birth are verified against the documents before anything is processed. Mismatches are flagged, not assumed.
The record is read
Every page routed and read — including scanned letters buried inside an otherwise digital export. Nothing is skipped silently.
You review and export
A cited chronology, split pre- and post-accident, with flags for the things that matter. Edit inline, then export to Word.
Scanned pages are the trap
A GP export can report a text layer on every single page while most of those pages carry their clinical content inside an embedded scan — behind nothing but a stamp.

Software that trusts a non-empty text layer reads that page as done, drops the letter inside it, and still produces something clean and confident. In one bundle we tested, 79 of 116 pages were affected.
NuvMed asks whether the text accounts for the ink on the page. Anything it cannot read confidently is put in front of you — never dropped quietly.
Built to be checked
- Every entry cites its source page. One click opens the original scan beside the extracted text.
- Dates are parsed, not guessed. Day-first and month-first conventions are inferred per document; anything ambiguous is flagged rather than resolved silently.
- You edit everything. Rewrite a line, exclude an entry, dismiss a flag. The export follows your edits, in your house format.
- UK-hosted, on your infrastructure. No third-party model endpoint. Records do not leave the system.
- Append-only audit trail of who viewed and changed what.
What it flags
Each flag names the records behind it, so you can open the page and judge for yourself. They are observations about what the records show — never conclusions.
Prior accidents
Earlier RTAs and trauma in the records, before the index accident.
Pre-existing symptoms
The same body region symptomatic beforehand — the apportionment question.
Subsequent accidents
A further accident afterwards, which may break the chain of causation.
Prognosis contradicted
The report says resolved in six months; the records show entries after that.
Late onset
A pleaded injury first appearing well after the accident date.
Unpleaded injuries
A new region appearing post-accident that nobody claimed.
